Managing chronic illness can be a full-time job and it takes a team working together. Abbott needed to understand how an app might help patients better manage their congestive heart failure, by facilitating communication between the patient and his/her care coordination team.
PointClear conducted a week-long, in-depth strategy session to identify the needed improvements to the mobile UI workflow; and proposed screens, a style guide, and overall design. PointClear UX experts provided on-going support and advisory services throughout the build of the product.
Redesigned and improved user interface of the mobile application helps streamline communications between patients and clinicians using the Abbott Merlin.net Patient Care Network website.
The app also syncs with the CardioMEMS HF System, which is the first and only FDA-approved heart failure (HF) monitor proven to significantly reduce HF hospital admissions and improve quality of life.
